Waiver

Due to the fact that physical exercise can be strenuous and subject to risk of serious injury, Score Hockey Range urges you to obtain a physical examination from a doctor prior to using any exercise equipment or participating in any exercise activity.

I agree that if I engage in any physical exercise or activity at Score Hockey Range, I am doing so entirely at my own risk.

You agree that you are voluntarily participating in Score Hockey Range activities and assume all risks of injury, illness or death.
This waiver and release of liability includes, without limitation, all injuries which may occur regardless of negligence as a result of your use of Score Hockey Range.
Score Hockey Range is not liable for our instruction, training, supervision or dietary recommendations and/or you slipping and/or falling during participation at Score Hockey Range.

You acknowledge that you have carefully read this “waiver and release form” and fully understand that it is a release of liability. You expressly agree to release and discharge Score Hockey Range and all affiliates, employees, agents, representatives, successors or assigns, from any and all claims or causes of action. You also agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against Score Hockey Range for negligence, personal injury or property damage.
